- Exhibit A (pricing matrix) - Star or circle the services you are most interested in using and

- Fax or Submit the Agreement for Service (5 pages) and both
pages of Exhibit A to 1.866.552.7326
Once we receive your Agreement, we will e-mail instructions along with your user id and
temporary password within 3 business hours.
Step 2:
Customize the attached Consent Form template and get each staff and volunteer
to fill out and sign.
Step 3:
Review our Multi-State Coverage document to ensure that coverage is adequate for your
